The solution
How it looks now:
- The orange line represents a social care customer being assessed at the highest level of need and getting better
- The purple box in the next diagram illustrates an all-too-typical care package, which carries on for a long time without review and is not designed to get the customer better
- This is sometimes called 'a maintenance package'
How it could look:
- The pink triangle in the lower graph represents the delivery of a totally personalised support plan
- The level of support should be reduced as the customer's desired outcomes begin to be met
- This shape is built into the support plan
- In this idealised case there is a potential saving represented of 66%
- Not all cases will follow anything like the orange curve but most are worth considering
